Update Your Details

To update details like your name, address, phone number or email address:

  • Use this NHS service to update or correct a missing or invalid mobile number or email address
  • Update or correct a missing or invalid mobile number or email address in your NHS account (through the NHS website or NHS App)
  • Submit an online consultation form on the Contact Us page on our website
  • Phone or visit the surgery from 10:00am to 6:00pm

Change of address

If you have moved, you must live within our catchment area to remain registered with our surgery.

If your new address falls outside our catchment area, you will have 30 days to register with a new GP surgery in your area. We are required to inform NHS England, and after this period, they will remove you from our patient list. NHS England will send you a letter confirming this.

You can use the Find a GP service to look for a surgery.

Changing your phone number or email address

You will need to show us identification before we can change your phone number or email address.

Changing your date of birth or name

You will need to show us proof that you have changed your name or date of birth. This could be a marriage or civil partnership certificate, deed poll or birth certificate.
